Self-harm is a highly relevant issue today, especially among adolescents, and its incidence has increased in recent years. However, there are very few studies related to this topic in Chiclayo, which is a cause for concern in the educational setting. Therefore, the objective was to determine the frequency of self-harm behaviors among adolescents attending a public institution in Chiclayo, 2025. The Self-Harm Questionnaire (SHQ-E) was used, along with a sociodemographic form. The design was non-experimental and descriptive. The population and sample consisted of 363 students from first to fifth grade of secondary school. It was found that self-harm behaviors are largely absent among adolescents from first to fifth grade of secondary school. Furthermore, females are more likely to exhibit these behaviors. Furthermore, the highest risk of engaging in self-harm is found among those aged 13 to 14, and single-parent families are more common in the study population.
