Introduction: Dentofacial aesthetics represents an essential aspect of modern dentistry, as patients seek treatments that enhance the harmony and attractiveness of their smiles key elements of self-expression and social acceptance. The objective of this study was to evaluate the influence of professional characteristics on the aesthetic perception of the smile among Peruvian dentists registered with the College of Dentists of Peru. Materials and Methods: An explanatory, cross-sectional, prospective, and observational study was conducted with a sample of 256 dental specialists from the fields of restorative and aesthetic dentistry, orthodontics and dentofacial orthopedics, and periodontics and implantology. Results: Aesthetic perception of the smile differed significantly according to the professional specialty and geographic region of the dentist (p < 0.05). Differences were observed in the detection of midline deviations and the vertical position of the incisors. Orthodontists and periodontists demonstrated a more accurate perception, whereas restorative and aesthetic dentists were more critical. No significant associations were found with sex or age (p > 0.05). Dentists from the Coast and Jungle regions provided more favorable evaluations (p = 0.038; p = 0.005; p = 0.001; p = 0.021). Conclusions: The aesthetic perception of the smile among Peruvian dentists was mainly influenced by the professional specialty and geographic region of origin, whereas sex, age, and years of professional experience showed no significant relationship.
